Understand Polyvagal Theory in Recovery

One framework that has become increasingly common in conversations about trauma and recovery is Polyvagal Theory. Developed by neuroscientist Stephen Porges, the theory is complex, and some of its specific physiological claims remain debated within the scientific community. Still, one of its broader ideas offers a useful way to think about recovery: our bodies are continually responding to cues of safety and threat, often before we consciously understand what we are reacting to.

In practical terms, this helps explain why someone can intellectually know they are safe while their body seems unconvinced. A person may be sitting in a quiet room, surrounded by supportive people, with no immediate danger present, yet still feel restless, guarded, irritable, or unable to settle. At other times, overwhelming stress may lead not to obvious anxiety but to withdrawal, numbness, exhaustion, or a desire to shut everything out. These reactions can be especially confusing in recovery because people often expect sobriety to make them feel calmer. Instead, removing drugs or alcohol can expose nervous system patterns that substances had previously masked.

The goal of nervous system healing is not to remain calm all the time. A healthy nervous system still responds to stress, anger, fear, excitement, and uncertainty. What changes is flexibility. The body becomes better able to mobilize when something requires action and, importantly, to settle again when the situation has passed. Recovery gradually expands that range. Instead of moving immediately from distress to substance use, a person begins developing other ways to experience discomfort, recognize what is happening, seek support, and return to a more regulated state.

Cortisol, Chronic Stress, and the Exhaustion of Early Recovery

The body’s stress response also helps explain why early sobriety can be so physically exhausting. When the brain perceives a challenge, it activates a coordinated hormonal response that includes cortisol. Cortisol is not inherently harmful; it is an essential hormone that helps regulate energy, blood pressure, immune activity, and our response to stress. Problems can develop when stress becomes prolonged and the body’s stress systems are repeatedly activated without enough opportunity to recover.

Addiction can contribute to that instability in several ways. Alcohol, opioids, stimulants, and other drugs affect the brain and body differently, but repeated intoxication and withdrawal can disrupt normal stress regulation. At the same time, the circumstances surrounding addiction may add additional strain through disrupted sleep, relationship conflict, financial instability, secrecy, trauma, poor nutrition, or the constant effort involved in obtaining and using substances. By the time someone enters recovery, their body may have been moving through periods of physiological and psychological stress for a very long time.

This is one reason fatigue in recovery deserves to be understood rather than immediately judged. Someone who has stopped using may genuinely want to rebuild their life and still find that their concentration is inconsistent, their patience is shorter than usual, or ordinary responsibilities require more energy than expected. The answer is not necessarily to push harder. Recovery often requires creating enough stability for the body to begin finding a more sustainable rhythm. Regular meals, movement, predictable routines, appropriate medical care, therapy, restorative sleep, and periods of genuine rest may seem less dramatic than other aspects of treatment, but they provide the conditions in which regulation can gradually improve.

Hypervigilance and Emotional Regulation After Addiction

For some people, nervous system dysregulation appears as hypervigilance: a persistent sense of being on alert even when there is no clear threat. Someone may constantly scan other people’s expressions, assume a change in tone means something is wrong, startle easily, struggle to concentrate in busy environments, or find it nearly impossible to fully relax. Hypervigilance is often associated with trauma and anxiety disorders, although feeling chronically “on edge” can have many causes and should not be self-diagnosed based on one symptom.

In recovery, these reactions matter because they influence how ordinary experiences are interpreted. A delayed text message can feel like rejection. Constructive criticism can feel threatening. A disagreement can produce an emotional response that seems much larger than the event itself. When the nervous system is highly activated, the brain has fewer resources available for reflection and flexible decision-making. This can make it harder to pause, consider another person’s perspective, or recognize that an uncomfortable feeling does not necessarily signal danger.

This is where emotional regulation and nervous system regulation begin to overlap. Emotional regulation is sometimes described as controlling feelings, but that misses the point. The goal is not to suppress anger, anxiety, sadness, or frustration. It is to develop enough capacity to experience those emotions without immediately becoming overwhelmed by them or needing to escape them. Therapy can help people identify what they are feeling and understand the experiences underneath those reactions, while grounding techniques, breathing practices, physical activity, consistent routines, and supportive relationships can help the body move through stress without relying on substances. Over time, a person begins to discover that an emotion can be intense without being permanent and uncomfortable without being dangerous.

Neuroplasticity: How the Brain Learns a Different Response

The hopeful part of this conversation is that the brain is not fixed. Neuroplasticity refers to the nervous system’s ability to change its activity and connections in response to experience. Addiction itself demonstrates this capacity in an unfortunate direction: repeated substance use strengthens patterns involving reward, cues, habits, and learned expectations. Recovery uses the same fundamental ability to build something different.

This does not mean that old addiction pathways simply disappear. Rather, repeated recovery experiences can strengthen competing patterns. Consider someone whose response to conflict was once almost automatic. An argument created distress, distress produced craving, and substance use offered immediate relief. In recovery, that sequence can begin changing. Conflict still produces distress, but the person recognizes the escalation earlier, takes space before reacting, calls someone supportive, discusses the experience in therapy, or uses another strategy that allows the feeling to pass. The first several times may feel awkward and deliberate. With repetition, the alternative becomes more familiar.

That is what makes seemingly ordinary recovery behaviors so significant. Keeping a regular schedule, following through on commitments, attending therapy, exercising, eating consistently, asking for help, spending time with supportive people, and tolerating difficult emotions without using are not merely signs that someone is “doing well.” They are repeated experiences through which the brain learns new expectations. Stress does not always have to end in escape. Loneliness can lead to connection. Anxiety can rise and fall. A craving can be noticed without being acted upon. Nervous system healing after addiction is therefore not a single event that occurs after a certain number of sober days. It is an accumulation of experiences that gradually changes what the brain expects from discomfort.

Sleep, Cravings, and the Body’s Need for Recovery

Sleep is one of the clearest places where the relationship between addiction and nervous system health becomes visible. Alcohol may make someone feel sleepy while disrupting normal sleep architecture later in the night. Stimulants can dramatically interfere with sleep and circadian rhythms, while opioids can also alter normal sleep patterns. When substance use stops, sleep does not necessarily normalize immediately. Some people experience insomnia, frequent waking, unusual dreams, daytime fatigue, or irregular sleep for a period of time as the body adjusts.

Improving sleep matters for reasons that extend far beyond feeling rested. Sleep supports memory, attention, emotional regulation, decision-making, and the brain’s ability to manage stress. A person who is chronically sleep-deprived is often more emotionally reactive and less able to tolerate frustration, which can make recovery more difficult. For this reason, persistent sleep problems deserve attention rather than being dismissed as something a person simply has to endure. A clinician can also help determine whether insomnia reflects withdrawal and recovery, an underlying sleep disorder, a mental health condition, medication effects, or another medical issue.

Cravings can also be understood partly through the lens of nervous system state. Not every craving begins with consciously thinking about drugs or alcohol. Sometimes the first signal is physical: tension in the chest, agitation, exhaustion, loneliness, anxiety, or the feeling that something is simply “off.” If substances repeatedly relieved those states in the past, the brain may quickly generate a craving because it remembers what once produced rapid relief. Recognizing this connection creates an important opportunity. Instead of treating every craving as an inexplicable demand for a substance, a person can begin asking what is happening underneath it. Are they exhausted? Frightened? Overstimulated? Lonely? Hungry? Angry? Experiencing a familiar environmental cue? The craving still needs to be taken seriously, but understanding the state surrounding it can make the response more informed.

Relationships and Rest Are Part of Nervous System Healing

Recovery is deeply personal, but nervous systems do not heal entirely in isolation. Human beings regulate through relationships as well as through individual coping skills. A calm conversation with someone trusted, the predictability of a healthy therapeutic relationship, time spent with supportive peers, or simply knowing that another person will remain present during a difficult moment can change how threatening that moment feels. This process is sometimes described as co-regulation, and it helps explain why connection is such a consistent feature of effective recovery support.

For someone whose relationships have historically been unpredictable, unsafe, or shaped by addiction, healthy connection may initially feel uncomfortable. Trust can take time. Vulnerability may activate the very nervous system responses a person is trying to regulate. Recovery provides opportunities to experience relationships differently: boundaries can exist without abandonment, disagreement does not have to become chaos, and asking for help does not have to result in shame. These experiences matter because they give the brain evidence that connection can be safe.

Rest belongs in this conversation for a similar reason. People in recovery sometimes feel pressure to compensate for the years addiction consumed by becoming relentlessly productive. Work, exercise, self-improvement, meetings, and new responsibilities can all be positive, but constant activity can also become another way to avoid stillness. Genuine rest asks the nervous system to do something different: stop performing, stop anticipating the next crisis, and allow the body to recover. That may be surprisingly difficult at first. Learning to rest without numbing, withdrawing, or escaping is itself a recovery skill.

Nervous System Healing After Addiction Is Part of Learning to Live Again

The purpose of nervous system healing is not to create a life without stress. No treatment can eliminate grief, conflict, disappointment, uncertainty, or fear, nor should that be the measure of successful recovery. A healthier nervous system is one that can experience activation without remaining trapped there. Stress rises and eventually falls. Difficult emotions can be felt without immediately becoming emergencies. A craving can appear without dictating what happens next. Rest can feel restorative rather than threatening, and relationships can become sources of connection rather than constant vigilance.

This is why long-term recovery reaches far beyond abstinence. Stopping alcohol or drugs removes a powerful source of harm, but recovery creates the conditions for the person underneath the addiction to regain flexibility, emotional capacity, physical stability, and trust in their own ability to move through life. Therapy, appropriate medical and psychiatric care, healthy sleep, movement, meaningful relationships, structure, nutrition, and rest all contribute in different ways. There is no single exercise or nervous-system “hack” that completes this work, and the process will look different depending on someone’s substance use history, physical health, trauma exposure, mental health, and individual circumstances.

What changes gradually is the relationship with discomfort. The nervous system no longer has to interpret every difficult feeling as something that must immediately be escaped. The brain gains new experiences of surviving stress without substances, and those experiences become the foundation for new patterns. Over time, recovery begins to feel less like constantly resisting an old life and more like developing the capacity to participate fully in a new one.

That is the deeper meaning of nervous system healing after addiction. Sobriety creates the opportunity for healing, but healing is what allows sobriety to become a life.
