This is the payment you regularly make for your insurance plan that keeps your plan active.

Out-Of-Pocket Maximum

This is the absolute most you’ll have to pay for covered services within the coverage year. After these costs are taken care of through deductibles, copayments, and coinsurance, your insurance provider will cover the remaining costs.

Policy Effective Date
